"Even though ACO providers say they are preparing for and assuming risk, the care delivery system is not advancing as quickly as the payment system reforms. In order for these payment models to be successful, providers need to change the way they deliver care."

The suburbs are home to about 44% of the population, and 38% of the uninsured population. Initially the uninsured rate was lower in the suburbs, but by 2015 that gap had closed, so that the number of those with coverage was about the same as those in suburban and urban areas. Previously, the uninsured rate was flat from 2005 to 2010, and then decreased, the study said. The Health Affairs study used national survey data from the Behavioral Risk Factor Surveillance System telephone survey of adults 18 to 64 years old for the years 2005 to 2015.
